Reductions
If you enroll for two or more Winter Courses simultaneously, you will receive a discount of 10% on each course.PhD students connected to Amaterdam Public Health research Institute (APH) will receive a 25% reduction on course fees. Please note that you need to fill in the form entitled ‘Promovendi APH discount’.

Application
The application deadline for courses in January and February is December 15. Enrollment is on a first come first serve basis.
Please read the cancellation policy before you submit the online application form. By submitting the online application form, you confirm that you accept the terms of cancellation. Once we have received your application, we will send you a conformation. We will send the invoice by email to the address (personal, work or invoice address) provided by you.
Initially, two-thirds of the available seats will be reserved for foreign students until November 15. Afterwards, remaining places may be assigned to Dutch students.
Payment
The student bears responsibility for payment, also in case the payment is made by the student’s employer.
For students from the U.S.A., it is not possible to have the invoice sent to their employer. The complexity of administrative procedures for the U.S.A. has proven to be problematic in the past. We will have the invoice sent to your private address (by email). You can discuss with your employer how to claim reimbursement.
We have also experienced similar issues with companies and hospitals from other countries. This is why we kindly request other international candidates to follow thiprocedure as well.
Cancellation conditions – courses
• Cancellations are only accepted in writing (by email to epdim@amsterdamumc.nl).
• Cancellation is free of charge up to six weeks before the start of a course;
• Up to two weeks before the start date, 50% of the course fee will be refunded.
• Less than two weeks before the start date, there is no right to a refund of the course fee.
• If you have registered but are unable to attend, a substitute participant is welcome at no extra charge, provided that you inform us in advance. Please ensure that he/she can provide proof of their identity.
Cancellation conditions – exams
• The exam fee is NOT refundable in case of cancellation by the student.
• A student who has cancelled his/her registration no later than 2 days (48 hours) before the start of the exam can register free of charge for the next exam opportunity.

This scheme is exclusively intended for APH PhD students
PhD students working on a research project embedded in the Amsterdam Public Health research institute (APH) can apply for a 25% discount on EpidM course fees. Please fill in the form ‘Promovendi APH discount’.
Requirements:
· If you qualify for the discount, download the “PhD students APH discount” form (see below), fill it in and send it to us along with your course application.· The discount is granted if the 1st supervisor declares that the PhD student will be awarded a PhD on an APH project.· The project must have been approved by the APH Scientific Quality Committee before the start of the course.· The APH-discount must be applied for before the start of the course.· You must provide the SQC number of the research project and the expected PhD date.
An APH project is a project approved by the APH Scientific Quality Committee and provided with an SQC number.
